Pressure Ulcers Elderly Assessment and Prevention

Pressure ulcers are common in almost any health care setting. Pressure ulcers are not confined to a particular age group, and the are very costly in terms of human suffering and resources. With the aging population this problem will only increase.  One possible reason that pressure ulcers are common in healthcare settings is that medication needed to increase patient comfort dull the senses, limiting sentience, and allowing pressure ulcers to form due to low blood pressure coupled with a lack of movement of the potential ulcerative tissue. This is considered a dominant cause in pressure ulcers elderly.

Most pressure ulcers elderly damage can be prevented with some common sense steps. The elderly should be assessed to determine how likely they are to get pressure ulcers and what steps will need to be taken to prevent pressure ulcers. The goal should be prevention by maintaining and improving tissue tolerance to pressure.

Skin assessment should be daily and such factors as bony structures, elbows, hips, and ankles should be given special attention. It is usually these prominent structures that receive the most pressure and therefore are most likely to develop pressure ulcers. Skin condition also plays a big role. Is the skin dry, cracking or damaged in any other way?

One of the best ways to prevent pressure ulcers is repositioning. People in wheel chairs are most prone to this problem. There are many repositioning devices that could be most helpful. There are wedges, pillows, foot guards, as well as amputee protection. Some of these repositioning devices are used in beds and wheel chairs to reduce the risk of pressure ulcers.

The primary risk factor is immobility and poor hygiene. The primary prevention of pressure ulcers is frequent repositioning and proper hygiene.
